5 quick Steps:

  1. Sign & Email Waiver & Video consent forms (click here) to
  2. Email a copy of your Sports (Medical) Physical to
  3. Submit payment of $45 per session (Cash, Cashapp & Zelle accepted). Checks are not accepted.
  4. Be on time. Please also have transportation ready before and after the workout. 
  5. Scheduling:  Coach Jay will then confirm the day and time of your workout with you!
